Initial 2010 code list released

CMS released the new diagnosis codes in the Federal Register as part of a larger proposed rule revising the Medicare hospital inpatient prospective payment system (IPPS). What was released is a listing of new, invalid and changed codes; there is no indication as to what Tabular List rules apply or what the changes will be, if any.

The biggest code changes appear to be related to venous embolism coding where about 20 new codes will be added to increase specificity. V codes should also impact home health, and a new anemia code will make documentation for anemia and cancer more critical

Final revisions and new codes usually are released in early June through the National Center for Health Statistics. In the meantime, you'll find the entire proposed list beginning on page 956 of the 1,228 page document.

Focus on venous ulcers: new code specificity coming

Take a look at the basics of venous coding now. You will certainly need to know how to use these codes correctly before the new changes take effect.

Ulcer
Stasis (leg)(venous) 454.0
With varicose veins 454.0
Without varicose veins 459.81
Inflamed or infected 454.2

There is no need for an ulcer code if a 454 code is used!

If there is no documentation of varicose veins, assign:
459.81, venous insufficiency
707.1x, ulcer of the lower limb

A male patient has swollen legs, cellulitis, draining ulcers on the calves and crusting. The diagnosis is stasis dermatitis and stasis ulcerations and cellulitis. Primary focus of care is the treatment of the cellulitis.
